With reference to Inquiry conducted vide Order No. VIG/Complaint (75)/2015/144 DT: 4/03/2016 provide following information:

1)      Inward number with date
2)      Certified copy of all correspondence carried out with reference to said Inquiry by the Inquiry Officer
3)      Certified copy of complete file prepared for said Inquiry including noting side
4)      Certified copy of all clarifications / explanations / correspondence received in reference to said Inquiry
5)      Present status of the said Inquiry
6)      If Complainant was called in the Inquiry, give certified copy of letter sent to him to be present in the Inquiry.
7)      If Complainant had attended the Inquiry, give date and time Complainant attended the Inquiry
8)      If any other officers had attended the said Inquiry, give name, designation, time and date that officer attended the Inquiry
